Understanding Corrective Actions: free short course

We have developed a free short course called Introducing Corrective Actions which provides a detailed look at the new corrective actions categories, the process of responding to corrective actions and guidance on completing a root cause analysis.

Redacting personal information in your assessments

Due to the highly sensitive nature of the personal information we receive during assessments, it is critical to ensure personal details are not identifiable. Redacting personal information not only protects the individuals in question but also the staff and the pharmacy itself.

Any evidence submitted with personal information visible will be rejected and will result in corrective actions issued to the pharmacy.

The following information must be fully redacted unless specified by the assessment question:

  • Patient names
  • Staff member names
  • Personal home addresses
  • Personal phone numbers
  • Personal email addresses
  • Medicare, Concession Card (e.g. Pensioner and Health Care) and Veteran Card details.
  • Photos identifying individual
  • Insurance policy numbers
